    Under Naloxone Guidelines, #3 Naloxone Administration, it should read: dilute naloxone ampule (400 mcg) with 10 ml normal saline - administer IV slowly at the rate of 0.5 ml (20 mcg) every 2 min until respiratory status improves or patient is responsive - maximum naloxone dose 20 ml (800 mcg) NOTE: technically, diluting a 400 mcg ampule with 9 ml normal saline will give the stated concentration. However, since most of the references we found stipulated 10 ml, we used that dilution to avoid confusion. How supplied: naloxone ampules of 0.4 mg (400mcg), 0.02 mg (20 mcg), 1 mg.

    Fentanyl transdermal (Duragesic) Conversion Chart: Janssen has revised the low conversion dose for the 25 mcg patch. Previous values were PO morphine 45-134 mg and IM morphine 8-22 mg. The currently advised doses are now PO morphine 60-134 mg and IM morphine 10-22 mg. On pages 4/5, this will increase all the values in the second column of all three charts (#tabs or cc's QD to convert to fentanyl 25 mcg patch). Since the conversion chart was already conservative, it is unlikely that using the old values would pose any serious risk of overdose.
